Public-site privacy boundary

The organization expression-of-interest form collects business identity, contact, facility-count and capability-interest information so authorized StoneHMIS operators can assess and respond to an enquiry.

Who can review it

Application records are stored in the StoneHMIS platform database and are available only through the protected Admin review pathway for authorized StoneHMIS developers and superadmins. Submitting does not create a tenant or user account.

Do not submit sensitive information

Do not send patient information, clinical narratives, prescriptions, credentials, tokens, private keys, database URLs or payment-card information through public StoneHMIS channels. The intake performs bounded automated checks but cannot make inappropriate submissions safe.

Retention and contact

Production activation remains blocked until the operating owner approves a retention/deletion period, privacy contact and applicant communication process. Until then, this implementation is for controlled validation and is not a production collection notice.

Operational data

Hosting and security services may process ordinary request metadata needed to deliver, rate-limit and protect the site. Public logs must not contain submitted clinical or credential information.
